i have been taking 4-5 tramadol a day for eighteen months for severe osteoarthritis sometimes with 1 hydrocodone depending on severity of pain

I'm a 63 year old female diagnosed with atrial fibrillation ten years ago. My cardio prescribed tramadol because NSAIDs are not good for my condition. I had knee surgery 3 years ago and have been taking tramadol. I know it's a synthetic opiod and addictive. Tramdol can be bought in The Philippines without prescription. Is it okey to continue taking it. What other pain killer can I take that can't give me provlems? I'm taking xarelto for my atrial fib. Thank you.
